fix(accessibility): tag needs-review issues, real dashboard links, and about 3x more issues per call - #525
Includes #526 (more issues per call), which was merged into this branch, so this PR now carries both.
Why
A PM review of the MCP on 2.2.8 found these problems in reading Accessibility reports:
Every issue looked confirmed. The report's CSV says:
which issues need review (the engine couldn't decide, so a person confirms them);
their review status;
which are best practices rather than WCAG failures;
the WCAG criteria each one checks.
The parser dropped all four, so an agent presented needs-review and best-practice findings as confirmed WCAG failures.
The scan's dashboard link didn't open the scan. It was built from the scan's name, unencoded: scanner.browserstack.com/site-scanner/scan-details/<name with spaces>.
App Accessibility build links opened on an error. The link had the build's project, name and number, but not the ?thBuildId= the dashboard's build page loads the build from.
Only 10–17 web issues per call. A mid-size report took a dozen calls and a large one about 30. There were three causes:
a page held only 10,000 characters of issue JSON;
each issue repeated its rule's text;
some HTML snippets ran to thousands of characters.
Nothing gave the whole report at a glance either.
What
Needs review, best practice and WCAG criteria
An issue the engine couldn't decide carries needs_review, set to its review status (unreviewed until someone confirms it).
Best practice and WCAG criteria are given on the issue's rule (see "Compact issues" below).
Every answer that reads a web report adds one line counted over the whole report. For example: "Of the 527 issues, 161 need review (a person has to confirm them) and 191 are best practices, not WCAG failures."
Needs-review issues a person has already confirmed don't count.
A count of zero is left out.
A note tells the agent what the tags mean. Only an issue with no needs_review and no best-practice rule is a confirmed WCAG failure.
The scan's own dashboard link
startAccessibilityScan takes the link from the scans list (scanUrl, on accessibility.browserstack.com with the scan's thBuildId) instead of building it from the name.
A failed scan's answer gets the link too, with its scan and run ids.
If the link can't be read, the answer gives the ids and no link.
App Accessibility build links
A build's link now ends in ?thBuildId=<build id>, the way the dashboard's own build list links it (apps/app-accessibility, useAutomatedTestList.js). The build page reads the build from that parameter, and without it shows an error.
This covers every App Accessibility build answer, including the "Report:" line of a build comparison. Manual reports (/report/<id>) were already right.
A page holds 30,000 characters of issues, about 8k tokens.
That's under Claude Code's 10k-token warning, and under its 50,000-character limit, above which a result is saved to a file instead of shown.
Compact issues.
Each rule's name, description, WCAG criteria, best-practice mark and usual fix are given once per page under rules.
An issue keeps its own fix or description only where it differs (colour contrast writes each element's values into its fix).
A rule's text counts against the page the first time the page names it.
Within a severity, issues are listed rule by rule, most common rule first.
HTML snippets over 400 characters are cut, with a note of how many more characters the CSV has.
Issues are sent as compact JSON.
The whole report first.
The first page, and a scan's answer, start with counts for the whole report: by severity, needing review, best practices, by rule (top 20) and by page (top 10).
Later pages skip the counts.
The CSV line says it holds every issue, and to download it when every issue is needed at once.
App Accessibility issue pages are unchanged apart from the link.

161 need review and 191 are best practices.
That scan's first reply is about 33.6k characters, roughly 9.6k tokens. That's close to Claude Code's 10k-token warning, but under its 50k-character limit.
dashboardLink returned the real scanUrl for two live scans, and nothing for an unknown one.
…e scan's real dashboard link
From a PM review of the MCP on 2.2.8:
- Every issue read as a confirmed failure. The report's CSV says which issues
need review and their review status, and which rules are best practices; the
parser dropped those columns. An issue the engine could not decide now carries
needs_review (its review status), a rule carries best_practice and its WCAG
criteria, and the answer says how many issues still need a person to confirm
them.
- fetchAccessibilityIssues handed out 10-13 issues per call. Each issue repeated
its rule's description and fix (over half its length), and one issue's HTML
snippet could be thousands of characters. A page now names each rule's text
once under rules (an issue keeps its own fix only where it differs), counts that
text against the page's length, lists a severity's issues rule by rule, cuts an
HTML snippet over 400 characters (saying how much the CSV has), and gives the
issues as compact JSON. The first page starts with the whole report's counts by
severity and rule, and the CSV link says it holds every issue.
- The scan's dashboard link was made from its name
(scanner.browserstack.com/site-scanner/scan-details/<name with spaces>), which
does not open the scan. It is now the scan's own link from the scans list
(scanUrl), for a failed scan too, with the scan's ids; when it cannot be read,
the answer gives the ids and no link.

The parser returns issue records separately from shared rule text. It adds review and best-practice counts, sorts issues by severity and rule frequency, truncates long HTML snippets, and accounts for rule text in page limits. The default page size increases from 10,000 to 30,000 characters.
Include summaries and rule data in report responses src/tools/accessibility.ts, tests/tools/accessibilityScanOutput.test.ts
Web-report and completed-scan responses include report summaries, rule data, CSV guidance, and explanations of review and best-practice tags. Tests cover summary wording, breakdown limits, and page behavior.
The scanner resolves a matching scan URL from the scans list. Scan responses use an available dashboard link or provide dashboard guidance by scan ID. App Accessibility build links include the encoded build ID.
browserstack/mcp-server#500: Changes accessibility CSV parsing and scan output using the same issue-count data flow.
browserstack/mcp-server#504: Adds App Accessibility build reports and dashboard links, which this change also updates.

Merge Risk: Minimal · up to bc4d9
The accessibility report and scan-link changes appear ready to merge after normal checks.
Security architecture risk: Low · up to bc4d9
The changes mainly affect report presentation and navigation. Issue records and shared rule text remain marked as untrusted, and dashboard lookup uses existing account credentials. The new severity summary is not similarly isolated; upstream validation of those labels remains uncertain, but an attacker-controlled path was not established.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The changed report data is returned to the invoking MCP client. The added dashboard lookup uses the configured BrowserStack account against a fixed API endpoint and selects one matching scan's URL. This request does not introduce a new credential type or permission grant; server-side authorization enforcement is outside the inspected code.
Security Findings and Attack Paths
inferred — A crafted severity label would reach agent-facing text outside the intended untrusted-data boundary. However, the inspected production flow obtains reports from authenticated export APIs, and no path establishing attacker control over that field was found. This is not sufficient to retain a verified prompt-injection concern.
Trust Boundaries and Controls
observed — The existing external-content control uses random delimiters and explicit instructions to treat enclosed content as data, not commands. Both changed web-report response paths apply this control to the combined issue and rule payload, preserving containment when shared rule guidance is extracted from individual issues.
Resilience and Maintainability Implications
observed — Dashboard lookup request failures and unsuccessful responses yield no link, allowing scan-ID guidance as a fallback. The added lookup occurs after polling reaches a terminal status and does not itself retry scan creation. Completed scans retain separate pending-export and ready-report response paths.
Hardening Proposals
proposed — Normalize severity labels to a documented finite set with an unknown fallback, or include the entire severity summary in the untrusted-data block. Either approach would avoid relying on an upstream field-validation guarantee that is not visible here.

The rest of the diff covers the report-wide counts, the 30,000-character page limit, the rule compaction in report-parser.ts and paginate.ts, the new dashboardLink method in scanner.ts, and the thBuildId parameter in app-report-fetcher.ts. The title is specific and relevant. It is long and lists three items, so it is a little overloaded, but it still summarizes the main change accurately.

… PR (kept on feat/a11y-issues-per-call)
This PR now covers two fixes only: needs-review / best-practice tagging and
the scan's real dashboard link. The issues-per-call work (rule text once per
page, grouping by rule, the page budget counting rule text, cut HTML snippets,
compact JSON, the severity/rule summary and the CSV wording) goes back to
main's behaviour here and is kept on branch feat/a11y-issues-per-call for a
separate PR.
Tagging now lives on each issue, since there is no per-page rules map:
needs_review (its review status), best_practice and wcag, from the CSV's
columns. Every answer that reads a web report says, over the whole report, how
many issues still need a person to confirm them and how many are best
practices, and what the tags mean. mostCommon is back in app-report-fetcher.

…e export runs, padded review status
- The note on the tags said every issue without needs_review is a confirmed
failure, which contradicted best_practice. It now says that only issues
with neither tag are confirmed WCAG failures.
- A scan whose CSV export was still being generated answered without the
dashboard link it had already read. That answer now gives it too.
- A review status written with spaces around it ("Confirmed ") read as a
different status and counted as still needing review, and an empty one
skipped "unreviewed". The status is trimmed first.

…ort's counts first
- A page of a web report now holds 30,000 characters of issues (about 8k tokens, under
Claude Code's 10k-token warning and 50,000-character limit) instead of 10,000.
- Each rule's name, description, WCAG criteria, best-practice mark and usual fix are said
once per page under rules; an issue keeps its own fix or description only where it
differs, and needs_review stays on the issue. The rule text counts against the page the
first time the page names the rule, a severity's issues are listed rule by rule (most
common rule first), HTML snippets over 400 characters are cut (saying how much the CSV

- The first page (and a scan's answer) starts with the whole report's counts: by
severity, needing review, best practices, by rule (top 20) and by page (top 10). The CSV
line says it holds every issue.
